We are honored that you have chosen our office for your medical care. For your first visit, please arrive 20 minutes earlier than your scheduled appointment time in order to complete initial paperwork.

Important things to know about your appointment:

Don’t forget to bring your insurance card and a photo ID.

We respectfully request 24-hour notice for appointment cancellations. Failure to do so may result in a $25.00 charge. Of course, illness, medical emergencies, and unusual work circumstances are considered.

If you arrive more than 10 minutes after your scheduled appointment time, you may need to be rescheduled for another day.

If your visit is related to a motor vehicle accident or Workers Compensation, inform the receptionist at check-in. Workers Compensation patients must have a claim number, claim address, and contact phone number.

A CD with a copy of your medical record can be provided for $15. If you wish a paper copy there is charge of $10.00 plus $.50 per page. A signed release form is required.

There is a $25.00 charge for all returned checks.
